We can offer support with things such as:
- Shopping collections or drop offs
- Provide timely food parcels for when you arrive home after your discharge
- Prescription collections
- Referrals and signposting to services
- Installing a key safe

What happens if you have been referred following a hospital discharge?
Once we have received your referral we aim to contact you within one working day to assess your support needs.
Please note that referrals are only monitored Monday-Friday 9.30am-12pm
