Register your interestAs a hospital discharge support volunteer, you will provide social, emotional and practical support to a patient being discharged from hospital.
The Discharge support project is the WECAN (West Essex Community Action Network) project , helping patients to be safely be discharged from hospital and return home.
As a volunteer you will provide social, emotional and practical support to a patient being discharged from hospital. This will improve their emotional health and wellbeing and help them feel less socially isolated.
You will be supported by our Discharge Support Community Hub Assistant and will be given training including basic safeguarding, confidentiality, lone working and health and safety. As a volunteer you will assist with:
· Helping patients packing belongings and getting ready to leave hospital.
· Taking patients to the hospital discharge lounge, ready to go home.
· Helping with shopping
· Picking up prescriptions
· Making wellbeing calls to check in with the patient
· Providing information about local groups

Driving patients home following their discharge
from hospital
