Register your interestThe Active Home Unit has been set up to help our patients start their journey home by supporting them in gaining back their independence and motivating them to re-learn the daily skills they require to enhance their standard of living.
The Active Home Ward includes support from external agencies such as the Red Cross, social care, therapies, community virtual wards to help reduce the deconditioning of patients at Broomfield and to optimise their independence whilst waiting on care.
This role will act as support to the ward team to engage patients with activities aimed at increasing mobility, independence, and functional wellbeing, whilst always maintaining the dignity and respect of our patients.
The role will require the need to be a positive, outgoing, and a creative individual that has the determination to fully support patients on their journey whilst in hospital.
The role involves working with patients, at varying levels of ability, on their individual goals set by the ward team and other care professionals and will have the opportunity to see first-hand the positive difference that can be made to patients in readiness for going home/or to step down beds.
